Risk Domain 4: Sexual Violence
Below are the report responses of an individual who has sexually offended. First, his MIDSA responses for each subcomponent of the sexual violence domain are presented as they appear in the report followed by an interpretation in red of each subcomponent. At the end of the subcomponents is an overall integrative interpretation in red.

The respondent reports that whereas his sadistic fantasies are frequent (> 86% of ACCs* and > 83% of ASOs**), he engages in only an average amount of sadistic behaviors (> 48% of ACCs and > 51% of ASOs). In contrast, his cruelty to animals, which is sometimes on sadism scales, is extremely high (> 99% of ACCs and > 93% of ASOs).

Consistent with his responses to the sadism scales, this respondent reports that whereas he has frequent expressive aggressive, angry, hurtful fantasies about women (> 99% of ACCs and > 89% of ASOs), his expressive aggressive behaviors are relatively rare (> 37% of ACCs and > 25% of ASO).

In contrast to his adult Sexual Sadism Fantasies, the respondent reports a low frequency of either fantasies or behaviors about sexually harming a child. He denies all items and reports the lowest amount possible.
Overview of the Entire Sexual Violence Domain
There are in all the samples we have tested (both offender and community) consistent high correlations between sadistic and expressive aggressive fantasies and their corresponding behavioral manifestations. Despite the respondent’s low acknowledgement of such violent behaviors, his high level of fantasies would warrant addressing these issues in treatment. He has admitted cruel behavior toward animals, so he has admitted some tendencies of behaviorally expressing his hostility. He reports that his sadistic fantasies are limited to adults.
