Risk Domain 1: Sexual Age Preferences
Below are the report responses of an individual who has sexually offended. First, his MIDSA responses for each subcomponent of the sexual age preference domain are presented as they appear in the report followed by an interpretation in red of each subcomponent. At the end of the subcomponents is an overall integrative interpretation in red.
This beginning part of his sexual experiences before 18 is followed by similar text, table, and list reports for each category of individual with whom he reported sexual contact (i.e., his second sister, brother, and young unrelated young females and males). This respondent’s reporting of childhood and adolescent sexual experiences reveals an extensive history of consensual sexual relationships with these five categories of individuals. His experience with young males was both more frequent and involved a greater variety of sexual behaviors (e.g., anal penetration) than other groups and foreshadowed the development of his ultimate sexual preference for 11- to 14-year-old boys.
This section on lifetime sexual preferences documents this respondent’s progression from polymorphous sexual attraction to both males and females to his final preference for 11- to 14-year-old boys, with a possible potential attraction to age-appropriate adult males.
This section documents the modus operandi of the respondent in his sexual behavior with youth. It delineates his sexual activities with his victims, his socializing with children, his relationships with his victims, the locations of the sexual encounters, the techniques he used to groom them, the rewards and promises he used, how he gained compliance, and what he did if he met resistance. His grooming and response to his victims suggests more emotional congruence with children than is obvious in his archival file.
The respondent indicates extremely high child sexual arousal both relative to the ACC* sample (i.e., T score > 99% of ACCs) and the ASO** sample (> 95% of ASOs). His Child Sexual Sadism is not as extreme but should demand some therapeutic attention (T score > 85% of ACCs and his percentile > 82% of ASOs).
The respondent reports extremely high exposure to both child (> 99% of ACCs and > 95% of ASOs) and homosexual pornography (> 99% of ACCs and 87% of ASOs). The former is not surprising, given his history. The latter suggests the possibility that helping him develop age-appropriate relationships with males might serve to meet some of his sexual needs and reduce his need to abuse 11- to 14-year-old boys.
The pattern of responding to these scales differs for those who have sexually abused children and those who have abused adults. Individuals who have abused children tend to be low on Masculine Adequacy and high on Anxiety with Women (and sometimes also high on Sexual Performance Anxiety). Those who have sexually abused adult women tend, in contrast to be high on Masculine Adequacy and low on both Anxiety with Women and Sexual Performance Anxiety. This respondent, consistent with his answers on the previous sections, scored low on Masculine Adequacy (> 11% of ACCs and > 10% of ASOs) and high on Anxiety with Women (> 97% of ACCs and >71% of ASOs). Both his Sexual Performance Anxiety and his Erectile Dysfunction scales are as low as they can be.
Consistent with this respondent’s other Sexual Age Preference domains, he scored high on Molester Cognitive Distortions (> 99% of ACC and 86% of ASOs) and low on the two cognitive distortions related to rape (Negative Masculinity/Toughness and Hostility toward Women).
Overview of the Entire Sexual Preference Domain
The seven sexual preference sections present a nuanced and detailed picture of the respondent’s sexual preferences. He began his sexual activity at age 9 and at first showed interest in both males and females. As he aged his preference focused more exclusively on males, and ultimately this preference settled on 11- to 14-year-old males. In early adulthood he attempted several heterosexual experiences, but quickly desisted. His high scores on Anxiety with Women and low scores on Masculine Adequacy are consistent with this. He had homosexual relationships up until his 30s and he reports extensive use of adult homosexual materials as well as child pornography. Adult homosexual relationships could serve as a potential alternative to 11- to 14-year-old boys for him, and this option should be explored in treatment. Currently, he is highly sexually aroused by children, and he rationalizes his sexual activity with them with extensive cognitive distortions about their sexuality. His modus operandi responses suggest that he has high emotional congruence with children and possibly views them as appropriate intimate as well as sexual relationships.
